The gun possibly may not be going into battery. I have had this happen on a M&P when I manually loaded a round in the chamber.

OP. what happens if you start with a full mag and empty chamber? If you rack the slide it should strip and chamber the first round and empty the mag as you continue racking the slide.
Do this with the muzzle pointed in a safe direction and KEEP YOUR FINGER OFF THE TRIGGER.

Carefully unload, field strip, and clean your pistol. Pay particular attention to the bolt face, the extractor pawl and the chamber. Excess firing, and a lack of cleaning can cause this problem. Brown Vinegar is good, at breaking down gunpowder residue, and soot. get the jumbo box of Q-tips, next time you're at the store.

At final inspection and lubrication, apply lubrication very sparingly. Q-tips, once again.
